Well Child Check & Childhood Immunization Schedule

Lawrence Pediatrics requires all patients to keep current on their vaccinations and well child checks. Patients who fall behind may be dismissed from the practice.

Birth: HBV, RSV (if born during RSV season)

Initial office visit: Newborn exam 1-3 days after discharge from hospital (or as recommended by hospitalist)

1 and/or 2 weeks: Infant Well Check as needed

1 month: Infant Well Check

2 Months: Well Child Exam and Immunizations: Vaxelis (DTaPIPV, Hib, HBV), PCV15, Rotavirus 

4 Months: Well Child Exam and Immunizations: Vaxelis (DTaP, IPV, Hib, HBV), PCV15, Rotavirus

6 Months: Well Child Exam and Immunizations: Vaxelis (DTaP, IPV, Hib, HBV), PCV15, Rotavirus 
*Note: influenza vaccine recommended
yearly beginning at age 6 months.

9 Months: Well Child Exam and Immunizations:Catch up as-needed.

12 Months:Well Child Exam and Immunizations:  MMR, Var, PCV15, HAV

15 Months: Well Child Exam and Immunizations: Catch up as-needed.

18 Months: Well Child Exam and Immunizations:Pentacel (DTaP, IPV, Hib), HAV

24 Month / 2 Years: Yearly Well Child Exam and Immunizations: Catch up as-needed.

3 Years: Yearly Well Child Exam and Immunizations: Catch up as-needed.

4-6 Years: Yearly Well Child Exam and Immunizations: Quadracel (DTaPIPV), MMRV

7-10 Years: Yearly Well Child Exam and Immunizations: Catch up as-needed.

11 Years: Well Child Exam and Immunizations:  TdapMCV4 HPV9 series

12-15 Years: Well Child Exam and Immunizations: Catch up as-needed.

16 Years and older: Yearly Well Child Exam and Immunizations: MenB, MCV4

*Influenza vaccine annually starting at age 6 months. Children ages 8 years and younger who get a flu vaccine for the first time will receive it in two separate doses at least 28 days apart for best efficacy.
